Abstract
This is a proposal for a practice innovation project using video decision aids to assist patients with a chronic illness with advance care planning. A model for evidence-based practice is applied to this research project. A literature review is completed. A clinical map is presented to guide implementation in the healthcare setting. Results of the project are imparted. The need for further research is explored and identified. The role of the advance practice nurse for this innovation project is considered.
