Abstract
Heart failure (HF), chronic obstructive pulmonary disease (COPD), acute myocardial infarction (AMI), and pneumonia are four targeted health conditions that have potentially avoidable frequent readmissions to the hospital in which payments are being reduced. This project was a quality improvement project to fulfill the requirements of a doctor of nursing practice project that took place at a cardiovascular specialty hospital in a Midwestern city. A brief motivational interviewing (MI) training session was used for staff to use in patients with HF, COPD, AMI, and pneumonia to increase enrollment in the organization’s care transitions program (CTP). The use of MI with CTP enrollment process was implemented for two and a half months and significantly increased the enrollment rate of staff trained to use MI. Patients with a diagnosis of HF were significantly more likely to enroll in the program compared to COPD, AMI, or pneumonia. The understanding and knowledge of MI improved through a brief training session and staff believed it helped empower patients as well as tailored the education to the patient rather than generalizing information for each patient. This project showed that a brief MI training session not only helped staff better understand MI, but also increased enrollment in a program that helps patients avoid hospital readmissions. Thus, MI can be implemented and improve outcomes for patients in an inpatient setting.
