Abstract
The purpose of this guide is to support the implementation of culturally tailored chronic disease self-management education programs in tribal communities in South Dakota. The guide builds on the Better Choices, Better Health® South Dakota (BCBH-SD) chronic disease self-management education (CDSME) workshops designed to support adults living with chronic conditions and caregivers to understand how healthier choices can improve quality of life, boost self-confidence, and inspires positive lifestyle changes. This guide outlines considerations to tailor the program to align with the unique traditions, culture, beliefs, and practices of each tribal community in South Dakota without compromising the core components of the BCBH-SD CDSME workshops. For health promotion programs to be impactful on tribal communities in South Dakota, programs such as this must be applied in ways that are culturally relevant to community traditions.
